璧山区某三甲医院十年血流感染分离菌分布及药物敏感性变迁研究

Abstract

目的:分析重庆医科大学附属璧山医院2015年至2024年血流感染分离菌分布特点及药物敏感性变迁。方法:用whonet5.6和SPSS 30软件对该医院血流感染分离菌数据进行分析。结果:收集到血流感染病原菌4 358株,其中阳性菌1 143株(26.2%),阴性菌3 046株(69.9%),真菌169株(3.9%)。前三位依次为大肠埃希菌1 747株(40.1%)、肺炎克雷伯菌876株(20.1%)、表皮葡萄球菌282株(6.5%);ICU和非ICU、儿童与成人分离菌有差异。该医院肺炎克雷伯菌ESBL从14.3%上升至25.0%,差异有统计学意义(P<0.05);ICU病区和非ICU病区MRSA检出率(40%VS25.4%)、肺炎克雷伯菌ESBL检出率(29.0%VS20.7%)、耐碳青霉烯肺炎克雷伯菌检出率(7.4VS2.7%)差异均有统计学意义(P<0.05)。结论:大肠埃希菌和肺炎克雷伯菌是该医院血流感染最主要的病原体。细菌耐药形势严峻,肺炎克雷伯菌的ESBL检出率呈显著上升趋势;ICU是耐药菌感染的高风险区域,其MRS、肺炎克雷伯菌ESBL及碳青霉烯类肠杆菌检出率均显著高于非ICU病房;病原菌分布在成人与儿童患者中存在差异。
