紧张型头痛与焦虑抑郁的关联机制及临床管理进展综述

Abstract
紧张型头痛(tension-type headache,TTH)是最常见的原发性头痛,严重影响患者的生活质量和心理健康。TTH患者中焦虑、抑郁的共病现象很常见,两者间存在复杂的双向交互影响机制。本文系统回顾了紧张型头痛与焦虑抑郁的流行病学特征、病理生理机制、临床表现及其彼此间的相互作用,分析了神经炎症、中枢敏化、人格特质与社会心理因素在这些关系中的效应。结合最新的临床研究,评述了合并焦虑抑郁的TTH患者的具体综合治疗策略,包括药物治疗、心理辅导以及非药物的治疗方法。借助对现有文献的分析总结,为临床诊疗提供理论依据与实践操作指引,从而促进TTH患者的整体管理及预后提升。
